It's true that painted horses are born that way, and don't suddenly start getting white patches growing in size.
Also possible that "Twilight" here is just heavily scarred. Chemical burns don't quite fit in with the "proud ethnic heritage" narrative though.
I've seen at least one or two, but I also live in a densely packed city. Much more opportunities to spot one.
A piebald horse does not have vitiligo, that's just a coat pattern.
To be fair, vitiligo is a lot more common in horses.

Some of these people eventually outgrow the lazy blobby CalArts anatomy, some don't. At least you don't see heterochromia as much any more. In the mid oughts babby's first Sanic OC (for male "artists"), or Inuyasha OC (for female "artists"), on Deviantart would always have two different colored eyes. ("She is cute, that is why she is called Boatlights-chan!")
i really don't get why they draw vitiligo so fucking often. vitiligo is unbelievably rare. have you ever seen anyone with it IRL? i haven't.
